Scheme Application Form

If you want us to deal with your complaint you will need to complete the Scheme Application Form. There are two ways of doing this:

Please contact us for assistance if:

• you need the form in a language other than English,
• you wish to receive the form in a different format,
• you have any other special needs.

Once you have submitted your application form, you should receive an acknowledgement from us within 14 days (28 days from overseas). If you do not receive an acknowledgement you should contact us again.
