MyOIA Online Complaint Form

If you want us to look at your complaint you will need to complete an OIA Complaint Form. The easiest way to submit a complaint is by creating a MyOIA accountMyOIA is a new secure online portal for providers and students (and their representatives) which provides an easy and more intuitive way to submit a complaint, and to track the progress of cases. Once you have created a MyOIA account you can save your form and come back to it at a later date as well as upload any supporting documents.

There are three stages of the form you must go through to finally submit your complaint to us successfully. First, you must fill out your details and your complaint. Secondly, you must read and agree to the declaration, and finally you can add your Completion of Procedures Letter.

Download the OIA Complaint Form

You can download the OIA Complaint Form as a digital PDF. This means you can complete it electronically, save it and e-mail it to us. You can also download the Welsh version of the OIA Complaint Form. Alternatively you can print the form and post it to us. Please keep proof of sending your OIA Complaint Form. You should receive an acknowledgement from us within 10 days. Please let us know if you haven't.

If you send us your OIA Complaint Form by email there is no need to send us an additional copy in the post.

It is easier to create and use a MyOIA account to submit your complaint to us but if you send your OIA Complaint Form by email or post, you can ask us to create a MyOIA account to track the status of your complaint online by ticking the relevant box in the section 'Completing the form'.
