Make a complaint

If you want us to look at your complaint you will need to complete a Scheme Application Form.

You can download the Scheme Application Form as a Word document. This means you can complete it electronically before printing and signing it.

Please contact us for assistance if you wish to receive the Form in a different format or have any other special needs.

Use our 'Complaints Wizard' to see whether your complaint is something the OIA can look at.

Please read the sections: 'How to make a complaint' and 'Tips about bringing a complaint to the OIA' before you send us your Form.

Remember to sign the Form!

We must receive your signed Scheme Application Form in this Office within three months of the date of the Completion of Procedures Letter; otherwise we may consider your complaint to be out of time. For example, if your Completion of Procedures Letter is dated 7 January, we should receive your Scheme Application Form by 7 April.

You can send us your Scheme Application Form by post, e-mail or fax.

If you e-mail us your Form you will need to scan it and attach the scanned copy to your e-mail so that we have your signature.

You should keep proof of posting or sending your Scheme Application Form.

Once you have submitted your Form, you should receive an acknowledgement from us within 14 days (28 days if you are overseas). If you do not receive an acknowledgement you should contact us again.
